Cambridge Health Alliance - Somerville Hospital
Somerville Hospital Primary Care is a local clinic that historically has provided health care to the underserved in the community. Their patient base includes a number of multi generation families as well as immigrants from Brazil, Haiti, and numerous Spanish-speaking countries from South America. This program is a new “patient-centered medical home” which engages the community in much deeper ways than traditionally done. Examples include offering tokens for local farmers’ markets to advocate for better nutrition, making house-calls as needed, and offering additional specialized services in severe psychiatric issues and HIV. Donated art will hang in the waiting areas, hallways, and exam rooms to welcome patients and staff to this new space dedicated to offering heath care to all.
